KYA Registration Form

The 2025 KYA Conference will be held November 13-15 at the Crowne Plaza Hotel (830 PHillips Ln, Louisville, KY 40209). Students will be dismissed from school on November 13 at the end of G block and will need to have their own transportation to the event. Students will need to be picked up from the event on November 15 at 11:00 am.

By typing my name (digital signature) below, I give permission for my child (named above), to participate in/attend the KYA Conference at the Crown Plaza Hotel (830 Phillips Ln, Louisville, KY 40209) November 13-15, 2025. I also understand that my child is required to follow all district and school policies during this school sponsored event. **Please note: If a student is not in good academic standing when grades are pulled for athletics on Tuesday, Nov. 4 OR has exceeded the threshold of 10% absences, she will not be permitted to attend KYA. Moreover, only partial refunds will be available due to YMCA guidelines.

I understand that this activity is a valid extension of Sacred Heart Academy's academic, co-curricular or extra-curricular program. In consideration for the making of arrangements for this activity by the Academy, I hereby release and save harmless any and all personnel of Sacred Heart Academy, Sacred Heart Schools, and the Ursuline Sisters from any and all injuries, loss, or other claims arising out of or resulting from this activity.

In case of emergency, illness, or accident to the above-named child, while on the off-campus school-sponsored field trip, I give consent to the nearest hospital and/or emergency medical professionals to render medical emergency care deemed appropriate by the hospital and/or emergency medical professionals. I also give consent to school personnel to take whatever action is deemed necessary in their judgement for the health of said child.
